Our most important priority is the patient’s health needs and making an appropriate and individualized plan for each of our patients. The Harrow Health Centre Inc.: A Family Health Team maintains a current, comprehensive knowledge of programs and services in Windsor and Essex County. We will recommend and assist you to connect with other health and service providers.

Oral Health Education

This program provides free oral health education to all HHC enrolled patients and members or the broader community. Research has shown that in communities where there is integration between medical and dental care providers, there are positive patient outcomes and reduced visits to the Emergency Department for emergency dental care.

 

With this pilot program available now on a time-limited basis until September 2022, we aim to provide a better healthcare experience by integrating both dental and medical care to improve your overall health.

Cancer Screening

This program provides preventative screening to all eligible patients for cervical, colorectal and breast cancers. By providing preventative screening to patients, we aim to increase survivability of our patients by detecting cancers early.

Immunization

Immunization

This program provides both childhood and adult immunizations to all patients. By providing, recording and promoting necessary immunizations to our patients we hope to eliminate preventable childhood diseases as well as preventable illness in adults. Annual flu clinics are offered to patients yearly and other immunization clinics may be offered from time to time, as recommended by the Public Health Unit.

Lung Health (ARGI)

This program provides support for those who are newly diagnosed with Asthma and COPD. A Respiratory Therapist will provide support and education to the patient regarding their condition. This program aims to reduce crisis treatment and visits to the Emergency Department by helping patients better manage ongoing needs through treatment plans developed by Certified Respiratory Educators. This is an award winning program, awarded the Minister’s Medal Award in 2013 and recognized at the European Respiratory Society 2016 International Congress with the Primary Care Respiratory Medicine Journal Award.

Pharmacy Review

Pharmacy Review

This program provides patients with comprehensive medication review and medication reconciliation with our on-site pharmacist to promote better understanding of each patient’s individualized medical care as requested by your doctor or nurse practitioner. The Pharmacist also plays a part in our Diabetes Education Service, our Smoking Cessation program and our Primary Care Memory Clinic.

Primary Care Memory Clinic

This program provides patients and caregivers with timely, comprehensive and high quality dementia care. An inter-disciplinary team, specially trained in caring for people living with dementia, will provide patients with an initial assessment of memory function, educate patients and caregivers on medication management, and provide an educational support system to caregivers answering any questions that they may have.

Smoking Cessation

Smoking Cessation

This program provides smoking cessation strategies and treatment to patients to encourage them to stop smoking. Our RPN and Pharmacist are certified in this program and will provide patients in the program with free Nicotine Replacement Therapy as well as counselling and support for program success.

Acute/Episodic Care

This service provides patients who are in need of acute/episodic care with same day or next day appointments. A Physician Assistant will be available weekly to provide patients with timely health care. This service aims to manage the demand for acute/episodic care and to educate patients on the appropriate use of urgent appointments.

Diabetes Education

Diabetes Education

This service provides education and awareness of diabetes to patients newly diagnosed with diabetes.  Our Dietician and Pharmacist will provide one-on-one consultations with the patient. Our team aims to provide comprehensive diabetes education, including a full explanation of medication and treatment plans as prescribed by your primary care provider. 

Our team has partnered with the Windsor-Essex County Community Health Centre to provide on-site Diabetes Wellness education to our rostered patients and also to members of our community.

These services are available to newly diagnosed diabetics, diabetics who are in need of more education on their condition, and to their families.

Dietician

This service provides patients and their families with education and guidance that will allow them to make healthy choices, assist patients in managing their weight, and assist diabetic patients in maintaining their blood sugar levels. A dietician will work closely with each patient and develop individualized dietary plans as well as provide each patient with support, counselling, and encouragement.

Holter/Loop Monitoring

This service provides care to all patients with cardiac symptoms requiring further investigation.

INR Services​

This service provides timely, point of care access to patients on warfarin. It aims to maintain good control of warfarin treatments received by patients.

Lab

Lab

This service is available daily to patients in the community. It provides patients with timely testing to acquire timely results and provides access to point of care testing such as INR testing, pregnancy testing, and urinalysis.

The Lab is open Monday to Friday morning starting 8:00am.

* Please note: Phlebotomy is a specialized skill and as a result this position is not back-filled during employee absences.  Please watch for notices. We apologize for any inconvenience this may cause.

Counselling Services

Patients in a crisis situation have access to an on-site counselling service.  This service is provided to our rostered patients and patients are referred to the Master Social Worker by our primary care providers.

Patients also have the option of attending the walk-in counselling services offered by Family Services weekly on Tuesday.  This service is free of charge, individuals are seen on a first come, first serve basis, and it is open to anyone in our community.

Online Booking

Online Booking

If you are looking for the flexibility of booking your own appointments on your time, take a few minutes at your next appointment to set up online booking with one of our medical office assistants. 

This secure service, supported by Medeo, is very new to our clinic and only some of our primary care providers are offering this option at this time. As the demand for this booking option grows, other providers will be adding their availability for the online booking. Stay tuned…

Ontario Telemedicine Network (OTN)

This service provides patients with virtual consultations with medical professionals through telecommunication technology to provide clinical health care in Ontario at a distance. The goal of this service is to close the distance gap that would otherwise negatively impact our patient’s ability to access care from specialists and services not consistently available in our area.

Currently the Harrow Health Centre supports the VON nursing station on Pelee Island using OTN. Our Physicians will accept Pelee Island patients and our team of professionals provide support to the VON clinic to assist in the care of these individuals.
