Please call the extension of the physician to whom you are enrolled to book an appointment with any member of our team, including Nurse Practitioners and Physician Assistant. If you get the voicemail, please leave a message. Someone will return your call as soon as they can.

The HHC has begun contacting enrolled patients to book Covid-19 vaccination appointments.
Local Public Health Units are leading vaccination planning and the HHC will follow their lead. The HHC has been advised of the roll-out plan for our Family Health Team by the Windsor Essex County Health Unit. Planning is actively underway and we will be in contact with priority groups and we will do so with WECHU guidance. Our Team will call you.
The way we operate over the next few months will look a little different, once again. To be able to offer vaccination clinics to our enrolled patients AND continue offering our regular primary care services, we will have to reduce the number of primary care providers available for same-day appointments. This change is temporary as we shift to do our part in the vaccination roll-out within our community. We apologize for the inconvenience this may cause from time to time. Every effort will be made to continue offering the level of service and same-day access you have grown accustomed to.
